On 3 April 2024 the FDA approved Zevtera (ceftobiprole medocaril sodium) for use.

Ceftobiprole medicaril, a pro-drug of Ceftobiprole, is used for the treatment of gram-negative bacterial infections. It is effective for bloodstream infections caused by MRSA, skin infections, and some pneumonia. It works best with resistant strains. The drug is effective against PBP2a, PBP2x and PBP2b, which gives it a broader perspective of antibiotic activity. Its mechanism of action is that it attacks specific penicillin-binding proteins. This is why it is considered one of the best options for treating resistant bacterial infections.
Indications for Use :
● Effective against Staphylococcal aureus bloodstream infections (infective endocarditis for adult patients).
● Effective against acute bacterial skin and skin structure infections for adult patients.
● Effective against community-acquired bacterial pneumonia for older children (3 months to 18 years of age) and adult patients.
Ceftobiprole is active against a broad spectrum of pathogens, such as:
• It effectively works on Staphylococcus aureus with PBP2a and MRSA.
• It acts on both gram-positive Streptococcus pyogenes and Streptococcus pneumoniae, E. coli, Haemophilus influenzae, penicillin-resistant strains with PBP2x and PBP2b, and Klebsiella pneumoniae.
LIMITATIONS – Does not target ESBL-producing Gram-negative bacilli (CTX-M, TEM, SHV) or bacteria with serine beta-lactamases.
Critically Important Points:
• Not applicable for VABP owing to the risk of mortality.
• Some antagonism was possible with vancomycin, daptomycin, gentamicin, and rifampicin.
• Administered via IV infusion over a period of two hours.
Side effects – nausea, diarrhea, headaches, rashes, injection conditions, increases in liver enzymes, and dysgeusia.
Precautions – Avoid using this medication for those who have a known history of severe hypersensitivity to ceftobiprole or any of its constituents.
